I have created this portal so that you can tell me a little bit about yourself and, if applicable, your child(ren) and family, before we meet.  You will also be able to sign-in to this portal to access your billing information and verify your upcoming appointment day and time.  

To get started, please sign-in on the right using the login name and password emailed to you; be sure to check the button next to "associate" if you are a parent of a child/teen under 18 and "patient" if you are 18 or over.  Once you are logged in, you will see a menu on the left-hand side of the page.  To start off, go to "Change My Password."  Later you will be reviewing documents and will be asked to sign them (e.g., to acknowledge agreement with the policies of my practice, etc.).  Your password will represent your signature, so please change your password before doing anything else on this portal and select a password that no one else knows so that only you have access to your account.  

Then, "My To Do List" details documents for you to complete.  Please go ahead and complete them before our first appointment.  

"Account Statements" will then list invoices and/or statements.

"Messages" will allow you to communicate with me via secure email.  (Please note that e-mail communication should not be used for treatment purposes or in a case of an emergency or for urgent requests or issues. I will make an effort to review and respond to any email communication as soon as possible; if you have not received a response to your email within 48 hours during business days or are concerned I may have not received your email, please follow-up by phone.)

Towards the bottom of the page, you will see your "Upcoming Appointments."

If you would like to return to this "Welcome!" page once you have logged in, you will find it in a column on the right-side of the page under Pages.
